Currently, there are two ways to connect to telehealth communications with your care provider:

1. Contact Your Provider Office - Representative will either provide link for access or alternative steps
2. Direct Link – Provided to you by staff representative

Once you’ve reached the log in screen, you must choose whether you are a CURRENT patient or a NEW patient.

Proceed to fill in the required fields below the welcome text:

1. First and Last Name
2. Date of Birth
3. Gender
4. Practice Selection (please refer to next section)

Practice Selection

If you logging in as a current patient, please type in the first few letters of your practice name. You will see a drop down appear reflecting the practice you are searching for. Please be sure to select the practice name.

If you are registering as a new patent, and are unsure about what practice you should enter, or do not have a provider you would like to see, please select “I don’t have one” within the practice selection drop-down.

Following a successful log-in, you will be presented with your contact/provider panel – from which you can select any provider or care coordinator (within the attributed practices) to begin either a chat or video with.

Online/Available Contacts

Please note: Communication will only be available when both the patient and care manager/provider are logged on. A patient is logged on if they show a green dot above the right side of their name or icon.

Once you click on a provider’s name, a new interaction panel will open on your right side. Here you will be able to begin communication by clicking on any of the two icons on the upper right corner of your screen. You can connect with your providers via chat or video call.

Chat

You will see all contacts available on the left panel and will be able to chat with the selected contact using the chat panel on the right.

Simply type the desired text into the text box at the bottom of your chat panel and click “SEND” to send a message.

Video Call

You will also be able to initiate a video call with the selected contact by selecting the video icon on the left panel. The receiving user will see a call icon flash alerting them of the incoming call. Once the call is answered you will see the video of the recipient and begin your video conversation.

Audio Call

As a patient user, you can receive phone calls to your cell phone/land line from your care provider. If your provider initiates an audio call with you, the call will go directly to the phone number you provided (if any).
